UNDERTAKINGS
1. I, _______________________________ (print name), confirm, after making all reasonable enquiries, that the below are all true and correct:

3. Administration of [deceased employee ]’s estate is not being obtained.
a) I am authorised to receive [Deceased employee ]’s Final Pay under section 65 of the Administration Act, that is, I am a person listed in section 65(2)(a) to (e) of the Administration Act, being: (please circle) b) the widow, widower, surviving civil union partner, or children of the deceased person, c) a surviving de facto partner of the deceased person, d) the persons beneficially entitled to the estate of the deceased person under the will or on the intestacy of that person, e) any person appearing to be entitled to obtain administration of the estate of the deceased person in New Zealand, f) any person related by blood or marriage or civil union to the deceased person who undertakes to maintain the children of that person who are minors or any of them; or g) any person who has and is exercising the role of providing day-to day care for any of the children of the deceased person who are minors.
4. I consent to receive the Final Pay, to be made to the following bank account which is held under my name: _____________________________ (insert).
5. I have attached to this undertaking a verified copy of photo identification and proof of bank account details.
6. I understand that if [employer] makes payment of the Final Pay to me on the basis of the information I have provided, it does so in good faith on the belief on reasonable grounds that the payment shall be valid against all persons whomsoever, and shall be absolutely discharged from all liability in respect of money paid.
7. I undertake that on receipt of the Final Pay, I will properly apply the Final Pay to [deceased employee ]’s estate.
